Hugh Baird College in Liverpool is recruiting for an EHCP & Transitions Coordinator to support learners with SEND, ensuring smooth transitions into College and coordinating EHCP processes. Based across multiple college sites in Liverpool, the role is part-time, permanent, 35 hours per week.

Qualifications

• Level 2 qualification in Maths and English.
• Experience working with EHCPs and SEND Code of Practice.
• Experience managing a high-volume caseload and meeting deadlines.
• Strong communication, report writing and organisational skills.
• Confidence working with Local Authorities, families and external stakeholders.
• Excellent IT skills and experience using databases and Microsoft Office applications.

Responsibilities

• Coordinate and manage EHCP annual reviews and consultations, ensuring statutory deadlines are met.
• Act as the main point of contact for Local Authorities, schools, parents/carers and external agencies.
• Support learners with SEND through effective transition planning and arrangements.
• Organise and facilitate annual review and transition meetings.
• Produce high-quality reports, consultation responses and learner documentation.
• Maintain accurate records and support High Needs funding and compliance processes.
