Job summary

Hugh Baird College in Sefton is seeking an EHCP & Transitions Coordinator to lead EHCP processes and ensure smooth transitions for learners with SEND. You will coordinate consultations, annual reviews and transition activities, working with Local Authorities, schools and families to meet SEND statutory requirements.

The role focuses on plan coordination, data accuracy and timely communication, supporting the Director of Learner Support and ensuring compliance with the SEND Code of Practice.

Qualifications

• Level 2 qualification in Maths and English.
• Experience working with EHCPs, SEND legislation and the SEND Code of Practice.
• Experience managing a high-volume caseload and meeting deadlines.
• Strong communication, report writing and organisational skills.
• Excellent IT skills and experience using databases and Microsoft Office applications.

Responsibilities

• Coordinate and manage EHCP annual reviews and consultations, ensuring statutory deadlines are met.
• Act as the main point of contact for Local Authorities, schools, parents/carers and external agencies.
• Support learners with SEND through effective transition planning and arrangements.
• Organise and facilitate annual review and transition meetings.
• Produce high-quality reports, consultation responses and learner documentation.
• Maintain accurate records and support High Needs funding and compliance processes.
