Job summary

Hugh Baird College in Liverpool is seeking an EHCP & Transitions Coordinator to support students with SEND, coordinating EHCP processes and transitions.

Working with the Learner Support team and external agencies, you will manage consultations, annual reviews, and liaise with Local Authorities to ensure SEND compliance.

Qualifications

• Experience coordinating EHCP consultations and annual reviews.
• Ability to liaise with Local Authorities, schools and families.
• Knowledge of SEND legislation and best practice in transition planning.

This is an exciting opportunity to play a pivotal role in supporting students with Special Educational Needs and Disabilities (SEND), ensuring smooth transitions into College and coordinating vital Education, Health and Care Plan (EHCP) processes. Working closely with the Director of Learner Support, Local Authorities, schools, parents/carers and external agencies, you will coordinate EHCP consultations, annual reviews and transition arrangements, ensuring compliance with SEND legislation and the SEND Code of Practice.
