Support Coordinator Yeovil, Somerset
Temporary, Part Time (28 hpw) until 31st March 2026
Salary £25,838 to £26,645 per annum, pro rata (depending on experience) and great benefits including Health Cash Plan

About Us

Home, a place where you belong. We are looking to recruit an enthusiastic and self-motivated Support Coordinator to work with customers experiencing difficulties in securing or maintaining accommodation. This role includes outreach support, hospital discharge, and targeted support work with customers in our accommodation services. As a Support Coordinator, you will provide support and advice to vulnerable and complex customers who have previously experienced homelessness.

Key Responsibilities
  1. Conduct needs assessments of at-risk and vulnerable customers to establish effective support plans.
  2. Carry out risk assessments, support planning, goal setting, and regular reviews.
  3. Provide and coordinate support, advice, and assistance to help customers maintain tenancies and access health and social services.
  4. Develop social networks, self-esteem, and independence among complex and vulnerable customers.
  5. Report quarterly on case studies and outcomes for customers.
  6. Collaborate with colleagues, organizations, and family/supporters to access and refer to specialist support services.
  7. Assist in furnishing and equipment acquisition through grants and charities.
  8. Support customers with daily living activities and facilitate moving to independent accommodation when appropriate.
Candidate Requirements
  • Passion to support customers in living their best lives with collaborative working and attention to detail.
  • Experience in creating person-centred support plans and supporting colleagues.
  • Experience in coordinating and assessing customer referrals.
  • Ability to work independently, remain calm under pressure, and demonstrate resilience.
  • Own a vehicle insured for business purposes; mileage will be reimbursed.
