Peterhead Community Hospital in Aberdeenshire is seeking an experienced Occupational Therapist (Band 6) to join the Community Rehabilitation Team on a part-time basis. The post focuses on delivering the Discharge to Assess model with travel across Buchan.

The role supports timely hospital discharge, safe home rehabilitation, and working within a multi-disciplinary framework to meet service needs. A full UK driving licence and car for business use are essential.

Responsabilidades

• Provide a priority response to facilitate timely hospital discharge.
• Provide a priority response for patients referred to virtual community wards.
• Work within a multi-disciplinary and multi-agency framework to ensure seamless care in the community.
• Manage a diverse caseload with a wide range of conditions and complex needs.
• Supervise and allocate work to OT support staff and contribute to student training.
• Play an active role in service development within your locality and the wider Health & Social Care Partnership.
