Job summary

Frimley Health NHS Foundation Trust in Slough is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital as a Discharge Facilitator. You will develop skills in assessment, care planning and multi-agency working to support safe, person-centred discharge planning.

You will coordinate complex discharges, identify barriers, work with patients, families and partner agencies, and help ensure timely patient flow and appropriate care settings.

Qualifications

• Registered nurse - NMC.
• ENB 998/ mentor prep or equivalent qualification.
• Broad clinical experience relevant to the post and experience in older people’s health.

Responsibilities

• Co-ordinate and plan complex discharges across clinical specialties.
• Identify barriers to discharge and help resolve them with wards and teams.
• Work with patients, families and partner agencies to ensure safe discharge planning.
• Promote efficient discharge processes to support timely patient flow.
