Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital. The role focuses on safe and effective discharge for patients, especially those with complex health and social care needs, ensuring discharge to appropriate settings in a timely manner.

You will coordinate complex discharges, support wards to identify and remove barriers, and work with patients, families and partner agencies to deliver person-centred discharge

Qualifications

• Experience coordinating and planning complex discharges across clinical specialties.
• Ability to identify discharge barriers and help resolve them.
• Experience working with patients, families and partner agencies for person-centred discharge planning.
• Proven ability to maintain patient flow and discharge processes in a hospital setting.

Job description

As a Discharge Facilitator, you will play a key role in supporting the safe and effective discharge of patients, with a particular focus on those with complex health and social care needs or where discharge may be delayed. You will contribute to the management of Discharge to Assess pathways, ensuring patients are supported in the right place, at the right time, with the right level of care.,

• Assisting with the co-ordination and planning of complex discharges across all clinical specialties.
• Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
• Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.
• Supporting timely patient flow through the hospital by promoting effective discharge processes.
• Strong communication and negotiation skills, with a flair for problem-solving.
• The ability to remain calm and effective under pressure in a fast-paced environment.
• A flexible, proactive approach, with the confidence to work both independently and as part of a multidisciplinary team.
• A commitment to high standards of patient care and collaborative working.
