Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital. The role focuses on safe and effective discharge for patients, especially those with complex health and social care needs, ensuring discharge to appropriate settings in a timely manner.

You will coordinate complex discharges, support wards to identify and remove barriers, and work with patients, families and partner agencies to deliver person-centred discharge

Qualifications

• Experience coordinating and planning complex discharges across clinical specialties.
• Ability to identify discharge barriers and help resolve them.
• Experience working with patients, families and partner agencies for person-centred discharge planning.
• Proven ability to maintain patient flow and discharge processes in a hospital setting.
