Job summary

Frimley Health NHS Foundation Trust is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital. You will develop assessment, care planning and multi-agency collaboration to support safe patient discharges.

As a Discharge Facilitator, you will coordinate complex discharges, work with wards, families and partner agencies, and strive to maintain timely patient flow and high standards of care in a busy hospital setting.

Qualifications

• Broad clinical experience relevant to the post and experience in older people's health.
• Experience of discharge coordination.
• Evidence of ongoing professional development.
• Experience of teaching.
• Experience of developing relationships with internal and external agencies and organisations.
• Experience of working across organisational boundaries within health and social care.

Responsibilities

• Assisting with the coordination and planning of complex discharges across all clinical specialties.
• Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
• Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.
• Supporting timely patient flow through the hospital by promoting effective discharge processes.
