Buckinghamshire Healthcare NHS Trust is seeking a motivated discharge coordinator to join the Discharge team. You will support planning, coordination and transfer of care from admission to safe discharge, under supervision, across hospital sites.

Responsabilités

• Support high-quality discharge and patient flow for complex discharge planning.
• Liaise with ward staff to identify patients needing complex discharge planning and coordinate referrals/assessments.
• Communicate with patients, relatives, carers, community staff, social services and others to develop an optimal discharge plan.
