Job summary

Qualifications

• A Registered Nurse with proven post-registration experience.
• Strong communication and negotiation skills, with a flair for problem-solving.
• Ability to remain calm and effective under pressure in a fast-paced environment.
• Flexible, proactive approach, with the confidence to work both independently and as part of a multidisciplinary team.
• Commitment to high standards of patient care and collaborative working.

Responsibilities

• Assisting with the co-ordination and planning of complex discharges across all clinical specialties.
• Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
• Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.
• Supporting timely patient flow through the hospital by promoting effective discharge processes.
