Job summary

Frimley Health NHS Foundation Trust in Slough is seeking an experienced Registered Nurse to join the Discharge Team at Wexham Park Hospital as a Discharge Facilitator. You will develop skills in assessment, care planning and multi-agency working to support safe, person-centred discharge planning.

You will coordinate complex discharges, identify barriers, work with patients, families and partner agencies, and help ensure timely patient flow and appropriate care settings.

Qualifications

Job summary

An exciting opportunity has arisen for an experienced Registered Nurse to join our dedicated Discharge Team at Wexham Park Hospital. This role offers the chance to further develop your skills in assessment, care planning and multi-agency working, while making a real difference to patients and their families.

As a Discharge Facilitator, you will play a key role in supporting the safe and effective discharge of patients, with a particular focus on those with complex health and social care needs or where discharge may be delayed. You will contribute to the management of Discharge to Assess pathways, ensuring patients are supported in the right place, at the right time, with the right level of care.

Main duties of the job

Key responsibilities include:

• Assisting with the co-ordination and planning of complex discharges across all clinical specialties.
• Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
• Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.
• Supporting timely patient flow through the hospital by promoting effective discharge processes.
