Responsibilities

• Support effective discharge planning for patients with complex health and social care needs across all clinical specialties at Frimley Park Hospital.
• Coordinate safe, timely, and appropriate discharge pathways, ensuring patients and families are fully informed and supported.
• Liaise with health and social care professionals to gather information, identify barriers to discharge, and develop person-centred plans.
• Manage competing priorities and respond to challenges in a fast-paced environment.
• Work flexibly and independently while contributing to the discharge team and maintaining accurate documentation.
