Job summary

Midlands Partnership University NHS Foundation Trust in Stoke-on-Trent is seeking a Discharge Facilitator to join the Discharge to Access (D2A) Complex Assessment team. You will assess and track patients needing a bed-based discharge in a timely manner, coordinating with the multi-disciplinary team and external agencies to ensure safe, efficient discharge.

The role works with the Patient Flow Team across hospital and community settings, supporting bed usage and patient transfers.

Qualifications

• Evidence of continuous development and acquisition of new skills and knowledge.
• NVQ Level 3 in Health Care or equivalent.
• Experience in a hospital or community setting, patient facing.
• Experience with clinical and non-clinical staff and relationship management.
• Experience using computer-based information systems in an office environment.
• IT proficiency with Microsoft Office and accurate keyboard skills.
• Standard IT literate with experience of IT systems/databases.
• Ability to work across the D2A footprint.
• Flexible approach and teamwork, adapting to last‑minute changes.

Responsibilities

• Coordinate discharge process from admission to discharge.
• Attend meetings, share information, escalate blocks or issues.
• Prioritise workload to meet objectives under Flow Team supervision.
• Demonstrate strong organizational and communication skills.
• Work with health and social care staff and Home First services understanding.
