Midlands Partnership NHS Foundation Trust is seeking a Complex Assessment Nurse within the Integrated Complex Assessment Team. The role involves assessing patients with complex needs for discharge, coordinating care plans and funding applications, and working with the MDT to ensure timely and safe bed-based assessments across the D2A footprint.

The post requires a Nursing Degree, UK professional registration, and the ability to travel across the footprint.

Qualifications

• Essential: Nursing Degree.
• UK professional registration required.
• Access to a vehicle and ability to travel across the D2A footprint.
• Desirable: Experience in discharge to assess or complex assessments.

Responsabilités

• Support MDT in delivering high quality care and effective care planning.
• Lead and participate in multidisciplinary team meetings.
• Undertake complex discharge assessments from admission to discharge.
• Provide guidance to junior staff and learners in care delivery.
• Coordinate timely assessments and funding applications under NHS frameworks.
• Maintain accurate records and ensure confidentiality.
