The Colman Centre Specialist Rehabilitation Service at East of England Community Health and Care NHS Trust is seeking an experienced Assistant Practitioner as a Discharge Co-ordinator. You will monitor and coordinate patient pathways from admission to discharge, and provide a troubleshooting support role to the multi-professional team in order to drive timely discharge.

You will work unsupervised when appropriate, develop strong links with the MDT, undertake audits as required, and support

• Experience in discharge planning and patient flow management.
• Strong administrative and time-management skills.
• Ability to work independently and as part of a multidisciplinary team.

• To work within the integrated team to facilitate early discharge from hospital.
• To work collaboratively across the pathway with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our patients.
