Job summary

Midlands Partnership NHS Foundation Trust is seeking a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) service in East Staffordshire. The role involves scheduling and allocating care calls on the Complex Discharge Tracker system, collaborating with Locality Teams to meet discharge timescales, and validating CDT updates across localities.

Ideal candidates will be enthusiastic, flexible, and capable of face-to-face care/support in the community.

Qualifications

• GCSEs or equivalent; Certificates/Qualifications in Administration.
• Ability to effectively maintain, update and sustain systems.
• Commitment to continuous professional development.
• NVQ Level 3 in Care or equivalent qualification or experience.

Responsibilities

• Review daily Home First referrals with Locality Teams and allocate care calls within discharge timescales.
• Sourcing and prioritising Home First referrals throughout the working day.
• Attend CDT training and briefing sessions as the system evolves.
• Provide care/support to community patients in their own homes on an adhoc basis.
