Job summary

Midlands Partnership University NHS Foundation Trust is seeking a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) service in East Staffordshire. You will schedule and allocate care calls using the CDT system, working with Locality Teams to meet discharge timescales.

The role requires flexibility, rotating shifts across 7 days and occasional face-to-face care in patients' homes. You should be a proactive team player, committed to safeguarding and effective communication

Qualifications

• GCSEs or equivalent in Administration required.
• Ability to maintain, update and sustain systems.
• Excellent communication skills.
• Commitment to continuing professional development.
• Significant experience in health / social care environment.
• Experience with legislation, regulation, policy and procedures.
• Experience delivering improvements to meet objectives.
• Ability to work collaboratively.
• Experience in work planning, allocator and coordinating care calls.

Responsibilities

• Review daily Home First referrals with Locality Teams and ensure appropriate care calls within discharge timescales.
• Source and prioritise Home First referrals throughout the working day.
• Attend CDT training and briefing sessions as the system evolves.
• Ensure CDT updates are accurate and corrected immediately.
• Provide care/support to community patients in their own homes on an ad hoc basis.
