Midlands Partnership NHS Foundation Trust is recruiting a Discharge Facilitator for the Discharge to Access (D2A) Complex Assessment team in Stoke-on-Trent. You will assess and track patients for bed-based D2A services, coordinating with the MDT and external agencies to ensure timely, safe discharges.

You will support the Patient Flow Team across hospital and community settings, gathering collateral histories and information to inform care needs.

Qualifications

• NVQ Level 3 in Health Care or equivalent knowledge.
• Experience in hospital or community settings with patient-facing duties.
• Experience using computer-based information systems and standard IT software.

Responsabilités

• Coordinate discharge processes from admission to discharge with the MDT.
• Attend meetings and record information, escalating blocks.
• Prioritise workload to meet discharge objectives under Flow Team supervision.
• Demonstrate strong organisational and communication skills in verbal and written form.
• Understand patient flow and local services supporting hospital admission avoidance.
