Job summary

Sirona care & health CIC in North Somerset is recruiting an Administrator for the Community Transfer of Care Hub. The role supports Discharge to Assess pathways to help patients leave hospital safely and access the right care in the community.

You will support clinical and administrative teams, handle enquiries, and maintain patient records within Sirona IT systems, contributing to timely discharges and improved patient experience.

Qualifications

• Functional literacy and numeracy level 3/ GCSE grade A-C or equivalent including English Language and Mathematics
• NVQ 3 Business and Administration or equivalent experience

Responsibilities

• Support the admin component within the Community Transfer of Care Hub (CToCH).
• Provide comprehensive admin support to health and social care professionals, hospitals, GPs, service-users and carers.
• Answer telephone enquiries in a friendly and professional manner.
• Maintain accurate patient records and data in Sirona IT systems (electronic patient record, databases, spreadsheets).
