Job summary

Sirona care & health is seeking an organised Administrator to support the Community Transfer of Care Hub (CToCH) in delivering Discharge to Assess pathways. You will aid hospital discharges, support patients and families, and collect data to demonstrate impact across hospitals, social care, primary care and community services.

Qualifications

• Literacy and numeracy at level 3 / GCSE A-C including English and Maths.
• NVQ Level 3 in Business and Administration or equivalent.
• Ability to work autonomously and manage tasks.
• Experience in a demanding office environment and meeting deadlines.
• Experience with IT clinical systems and Microsoft Office.
• Call handling experience; healthcare experience preferred.
• Discretion and confidentiality with sensitive information.
• Strong workload prioritisation and time management.

Responsibilities

• Support discharge to assess pathways and hospital discharge processes.
• Assist clinical and administrative teams with data collection and reporting.
• Coordinate with hospitals, social care, primary care, community services, and ICB.
• Support patients and families throughout the discharge journey.
• Help develop new ways of working to ensure safe, timely discharges.
