Job description
Overview

This post has been established to support the seamless and effective running of the diabetes in pregnancy service. The midwife will work with the multidisciplinary diabetes in pregnancy team in both face-to-face and virtual settings, harnessing technology to support women and their families diagnosed with gestational diabetes.

Responsibilities
  • Utilise the GDm-Health app and onboard eligible women in how to use it.
  • Follow up women according to local and national diabetes guidance and support them to engage with appropriate services, make informed choices and prepare for parenthood.
  • Support the diabetes antenatal clinic, the virtual diabetic clinic and the GDm-Health app dashboard; audit the service and maintain and update the database.
  • Support any research projects associated with diabetes in pregnancy.
  • Work in partnership with the lead midwife for the service, supporting women to achieve positive pregnancy outcomes that are personalised, safe and in line with evidence.
  • Identify and explain the GDm-Health app to eligible women and ensure use is monitored via the dashboard in the virtual setting.
  • Organise follow-up visits and triage the women who are onboarded onto the GDm-Health app.
  • Communicate complex information to patients regarding diabetes in pregnancy to ensure understanding of the impact on pregnancy.
  • Communicate effectively with staff involved in the antenatal clinic and diabetes service.
  • Provide midwifery support to the diabetes in pregnancy service.
