A leading hospital trust in the UK seeks a professional to provide education, support, and diabetes care for inpatients. The role involves managing a caseload in collaboration with a multidisciplinary team, ensuring high standards of patient care and effective communication during hospital-to-home transitions. Ideal candidates will have experience in diabetes care and excellent interpersonal skills. This position is crucial in promoting integrated care for patients with diabetes.
Provide education, advice and support to inpatients with diabetes and their carers, under the supervision of the Diabetes Specialist Nurses.
The role includes supporting patients in a variety of clinics such as pregnancy and diabetes clinics and new patient clinics.
