Job summary

Lincolnshire Partnership NHS Foundation Trust in Lincoln is seeking a Physical Health Care Practitioner (Band 6) for CYP Urgent Care. You will manage physical health needs, lead within a multidisciplinary team, and support service development across the week, with a negotiable base and flexible working options to promote work-life balance.

You will work within an MDT of clinicians to support high-risk children and young people, ensuring care is evidence-based and aligned with trust policy.

Qualifications

• Registered Health Practitioner with registration in RN/OT/PT.
• Teaching qualification ENB 998 or equivalent.
• Evidence of continued professional development.

Responsibilities

• Devise, deliver and evaluate care for physical health needs via assessment and care plans.
• Provide clinical leadership and MDT collaboration with patients, carers and colleagues.
• Support development and drive improvements in physical health clinical practice.
• Ensure practice aligns with Trust policy and improves access to physical health.
• Support integration across CCETT, EDS and MHLS to ensure access for all patients.
