• Comprehensive assessment, care and safety planning for urgent care patients.
• Accurate clinical documentation in line with professional codes and Trust policy.
• Develop and maintain specialist clinical skills in mental health assessment and treatment.
• Triage and assess patients to ensure appropriate care pathways.

Responsibilities

• Provide comprehensive assessment and safety planning for patients in Urgent Care.
• Ensure timely clinical documentation.
• Develop and maintain specialist clinical skills in mental health care.
• Receive, triage and assess patients ensuring appropriate care and support.
• Advise on management of mental health issues with other professionals.
• Assess, investigate, treat and discharge or refer patients per guidelines.
• Feedback outcomes to department leaders and implement management plans.
• Support Clinical Lead in service development and workload management.
• In absence of Clinical Lead ensure timely referral and workload allocation.
• Prioritize daily workload based on risk and patient needs.
• Participate in team meetings and contribute to new care models using QI.
