Job summary

Pennine Care NHS Foundation Trust is recruiting a Band 6 Mental Health Nurse for the Crisis Resolution Home Treatment Team in Tameside and Glossop. The role involves holistic assessments, risk management, and delivery of evidence-based therapies in the home environment.

You will work within an MDT, coordinating with psychiatrists, psychologists and other colleagues to support adults with severe and enduring mental health needs, providing safe, compassionate care in the community.

Qualifications

• Registered Mental Health Nurse with current NMC registration.
• Experience in crisis resolution/home treatment or similar mental health settings is desirable.
• Successful completion of Preceptorship and ongoing professional development.

Responsibilities

• Undertake holistic mental health assessments and formulate risk plans.
• Provide person-centred care plans and evidence-based interventions.
• Collaborate with MDT members and external agencies to ensure continuity of care.
• Maintain accurate, timely records using PARIS and ensure confidentiality.
