Job summary

East London NHS Foundation Trust is seeking a dedicated professional to join the Crisis Resolution Home Treatment Team (CRHTT). The role focuses on rapid assessment and home-based treatment for adults in mental health crisis, with emphasis on accurate risk formulation and safeguarding.

You will work within a multidisciplinary team, assess referrals, provide gatekeeping, and support discharge planning. The post requires professional collaboration across teams and agencies to promote recovery.

Qualifications

• Assess and manage risk for patients in crisis using baseline physical and mental state measures.
• Provide psycho-social interventions to support recovery and reduce readmission.
• Deliver training to clinical teams on managing mental health crises and related care.

Responsibilities

• Work within a multidisciplinary service to assess referrals from GPs, CMHTs, and acute wards.
• Be part of the home treatment or assessment team within the Crisis Team.
• Provide gatekeeping and feedback based on CRHTT criteria.
• Conduct comprehensive risk assessments including physical health considerations.
• Offer expert advice on management and interventions to minimise risk and promote recovery.
• Support acute hospital wards and facilitate safe discharge or leave to reduce length of stay.
• Utilise psycho-social methods to improve mental health outcomes for patients.
• Act as shift co-ordinator, assigning tasks for the day.
• Adhere to lone worker policies and supervise medication administration when required.
