Pennine Care NHS Foundation Trust is recruiting for a Mental Health Nurse to join the Crisis Resolution Home Treatment Team in Greater Manchester. Qualifikationen

• Registered Mental Health Nurse with current NMC registration.
• Completion of Preceptorship or equivalent.
• Experience in community mental health or crisis services is desirable.

Aufgaben

• Conduct comprehensive mental health assessments and risk assessments.
• Develop and monitor collaborative care plans with MDT.
• Provide evidence-based therapeutic interventions.
• Educate patients, families and carers; promote recovery and independence.
• Maintain accurate records in line with Trust policies (PARIS).
• Liaise with MDT and external partners to coordinate care.
