Job summary

Pennine Care NHS Foundation Trust is recruiting for a Band 6 mental health practitioner to join the Crisis Resolution Home Treatment Team in Ashton-under-Lyne. You will work within an MDT to deliver holistic assessments, risk assessments and care planning in the community.

You will provide evidence-based interventions, support patients in crises and promote re-enablement, with opportunities for role development as part of service transformation.

Qualifications

• Registered Mental Health Nurse with current NMC registration.
• Preceptorship completed and up-to-date professional registration.
• Experience in community mental health settings is desirable.

Responsibilities

• Undertake timely comprehensive mental health assessments.
• Identify and implement effective risk management plans.
• Provide person-centered well-being care plans using SMART goals.
• Promote choice and service user involvement in care planning and risk management.
• Maintain high quality records and reports in PARIS system.
