Job summary

Rotherham Doncaster and South Humber NHS Foundation Trust is recruiting a Crisis Resolution Home Treatment Practitioner (NHS Band 6) to provide urgent mental health care in the community from Scunthorpe. The role includes 24/7 service delivery, triage, care planning, risk management and acting as a clinical lead within the MDT.

You will deliver home-based treatments, work with crisis houses and guide early discharge from inpatient wards where clinically indicated, ensuring safe, recovery-focused

Qualifications

• Post-registration experience in mental health settings.
• Experience working in MDT environments.
• Experience in risk assessment and care planning.
• Experience of safeguarding and crisis management.

Responsibilities

• Provide 24/7 clinical service focusing on urgent mental health care.
• Triage referrals to determine priority and pathway.
• Deliver nursing care, including comprehensive assessment and care planning.
• Perform gatekeeping assessments for admissions and crisis housing.
• Lead on admissions care and support early discharge when appropriate.
