The CRHTT work within the MDT including psychiatrists and psychologist to support residents with severe and enduring mental health conditions. We aim to provide outstanding care to the people of Tameside and Glossop and listen to improve, in response to the needs of the population we serve.

The CRHTT work with people aged 18 and over with severe and enduring mental health needs and support those in crisis within their own homes as an alternative to hospital admission. The CMHT works closely with stakeholders, providing a multidisciplinary approach to the care provided.
Mental health practitioners are responsible for providing holistic assessment, interventions, risk assessments and care plans for service users referred to the team.
