Job summary

East of England Community Health and Care NHS Trust is seeking a 12-month fixed-term community triage nurse to assess referrals from Norwich, prioritise care, and support patients by telephone. The role is office-based with visits to homes and clinics, seven days a week from 8am to 6:30pm, with some weekend work.

Current NMC registration is required, but triage experience is not essential as full training is provided.

Qualifications

• Current NMC registration is required.
• Experience in a multidisciplinary team.
• Experience delivering care plans for patients with multiple conditions.
• Excellent communication skills in written and spoken English.
• Good observational and reporting skills.
• Good time management and ability to work in busy environments.

Responsibilities

• Evaluate referrals and triage to appropriate service.
• Triage planned nursing visits and urgent community responses.
• Assess, plan and deliver care packages.
• Deliver clinical care at patients' homes or clinics.
• Document triage notes and maintain records for audits.
• Use IT systems to process referrals.
• Support and coach patients by telephone.
• Liaise with health professionals and partners daily.
• Support early discharge and prevent unnecessary admissions.
