Job summary

Norfolk Community Health and Care NHS Trust is recruiting a Community Nurse for the West Norwich Community Nursing Team. The role is based at Norwich Community Hospital with shifts from 8am to 6pm, including weekends and bank holidays.

You will join a supportive team of nurses, HCAs, practitioners and nursing associates delivering care at patients’ homes and in clinics. The post welcomes experienced and newly qualified nurses, offering a supernumerary period, training and supervision, and

Qualifications

• Current registration as a nurse with the NMC.
• Experience in community/nursing care is desirable.
• Ability to work in a multi-disciplinary team and provide patient-centred care.

Responsibilities

• Assess, prescribe and deliver care packages for patients.
• Support early discharge from hospital within the integrated team.
• Prevent unnecessary hospital admissions by proactive community care.
• Collaborate with other health professionals and agencies for integrated services.
• Process referrals through clinical assessment and prioritisation.
• Provide clinical assessment for incoming nursing referrals.
