Job summary

The Dudley Group NHS Foundation Trust is seeking an experienced Clinical Triage Nurse to join the Care Navigation Centre on a 10-month contract. You will perform telephone triage, provide clinical advice and signposting, and support urgent care across the health economy.

Working 8am–9pm, seven days a week, you will collaborate with the Urgent Community Response Team to prevent hospital admissions and to direct patients to the most appropriate community services, drawing on strong nursing

Qualifications

• Registered nurse with current registration.
• Evidence of Band 6 professional development.
• Knowledge of Primary Care working.
• Post registration experience including mentoring and project work.

Responsibilities

• Provide clinical support to prevent unnecessary hospital attendances.
• Refer to the most appropriate service for patient needs.
• Take telephone referrals from GP, ambulance service and health professionals.
• Visit patients at home with support from Medical Lead/Senior Practitioners when required.
• Prioritise workload with excellent communication and IT skills.
