East of England Community Health and Care NHS Trust is seeking a Band 5 Community Nurse to join Norwich-based nursing teams. The role involves nursing care and interventions across Norwich Place, visiting patients at home and in clinic settings, with shifts from 8am to 6pm including weekends and bank holidays.

A supernumerary period and strong supervision are offered to support professional development. Recent and newly qualified nurses are encouraged to apply.

Qualifications

• Must be registered with the NMC as a qualified nurse.
• Experience in community or home-based nursing is desirable but not essential.

Responsabilités

• Assess patients and deliver nursing care and interventions.
• Coordinate care with the integrated team to support early discharge from hospital.
• Liaise with patients, families, and other health professionals to provide seamless care at home and in clinics.
