Sheffield City Council is seeking a Home First Community Support Worker to provide personal and practical care to adults in their homes across Sheffield. You’ll work flexibly to help people stay independent and safe, including after hospital stays or changes in care needs.

You will join a committed team, with responsibilities spanning safeguarding, collaborating with families and health professionals, and supporting people to live confidently at home, including evenings and weekends.

Qualifications

• Experience in care and support work, including paid, volunteering or caring roles.
• Willing to undertake Health and Social Care training as required.
• Ability to work flexibly to meet individual needs and maintain independence.

Responsabilités

• Provide personal care and practical support in clients' homes.
• Support individuals to achieve goals in their support plans.
• Encourage healthy lifestyles and nutritional needs.
• Respond to urgent support requests and changing care needs.
• Support safe, independent living and liaise with families and health professionals.
• Contribute to safeguarding and welfare of vulnerable adults.
• Work evenings and weekends as part of a seven-day rota.
