Job summary

An exciting opportunity* has arisen to join our vibrant Adult Community Nursing Service. Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high-quality services to residents of the City and Hackney boroughs.

We are seeking an enthusiastic Band 5 nurse to fill a full-time, permanent post. The Adult Community Nursing Service is divided into 8 neighbourhood teams. Successful candidates will be asked to work within one of these teams.

*Please note - this vacancy is not suitable for Overseas Registered Nurses

Main duties of the job
  1. The post holder will practice within a multidisciplinary team and provide care to patients within their own homes and community settings.
  2. Assess, plan, implement, and evaluate care needs under the supervision of Clinical Nurse Specialists, Practitioners, and District Nurses.
  3. Deliver a high standard of holistic nursing care, promoting health education and independence.
  4. Take responsibility for personal actions when delivering clinical care, maintaining competency and skills as required.
