Join Our Adult Community Nursing Service at Homerton Healthcare NHS Foundation Trust

An exciting opportunity has arisen to join our vibrant Adult Community Nursing Service. Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high-quality services to residents of the City and Hackney borough.

We are seeking an enthusiastic Band 5 nurse to fill a full-time permanent post. The Adult Community Nursing Service is divided into 8 neighbourhood teams. Successful candidates will be asked to work within one of these teams.

Please note: this vacancy is not suitable for Overseas Registered Nurses.

Key Responsibilities

1. Practice within a multidisciplinary team, providing care to patients in their homes and community settings.
2. Assess, plan, implement, and evaluate patient care needs under supervision of Clinical Nurse Specialists, Practitioners, and District Nurses.
3. Deliver high standards of holistic nursing care, promoting health education and independence.
4. Maintain competence and skills, taking responsibility for clinical actions.
