This position is available in the Thame/Princes Risborough area to become a Community Nurse within a newly formed team. We are seeking highly motivated, effective team players committed to providing high-quality nursing services in the District Nursing field. As a community nurse, you will be supported to develop into an autonomous practitioner, caring for patients with complex healthcare needs in their own homes.
Community nursing is a specialised practice area. As a Community Staff Nurse, you will work within a locality team comprising District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, and Healthcare Assistants. Your role will involve meeting the needs of complex patients in the community and preventing admissions to acute settings. Responsibilities include medication administration, end-of-life care, managing deteriorating patients, complex and simple wound care (including negative pressure wound therapy and compression therapy), venepuncture, and utilizing advanced assessment and clinical skills. We provide holistic care to individuals with various conditions in their homes and residential care settings. Community nurses collaborate closely with social care and other primary health professionals such as GPs, Community Therapies, and specialised services like Tissue Viability, Diabetes, and Heart Failure.
