Job summary

NHS National Services Scotland is recruiting an assistant practitioner to join our Community Rehabilitation Teams. You will work with Occupational Therapy and Physiotherapy staff to deliver therapeutic treatment programmes in service users' homes, supporting early discharge from hospital.

You will hold your own caseload, assess, progress and discharge autonomously within agreed parameters, and may work across local community sites.

Qualifications

• HNC/HND in Health and Social Care or SCQF Level 8.
• Experience working as a therapy support worker.
• Experience working with older people.
• Ability to travel throughout the area.

Responsibilities

• Deliver therapeutic treatment programmes in service users' homes with OT/PT staff.
• Hold and manage your own caseload, assess, progress and discharge autonomously within parameters.
• Work across community sites and participate in supervision and PDP.
