Job summary

Nottingham CityCare Partnership CIC is recruiting a full-time Band 4 Stroke Practitioner for the Community Stroke Service in Nottingham. You will assess and support rehabilitation in patients' homes, care homes and clinics as part of a large multidisciplinary team, managing your own caseload and contributing to audits and service development.

We seek compassionate, motivated individuals with health/social care experience who can work across settings and settings with self-motivation,

Qualifications

Educated to Foundation degree or equivalent in a health-related discipline.
NVQ / QCF Level 3 in Health and Social Care (Desirable).
IT literate with ability to use standard software and electronic records.

Responsibilities

To work within the multi-disciplinary Community Stroke Team to provide assessment and rehabilitation for patients who have had a stroke.
To deliver an integrated rehabilitation service by providing generic assessments of social care and health needs across professional boundaries.
To accept responsibility for designated caseload, working independently and promoting independence for patients after stroke.
To potentially co-ordinate and lead exercise classes for stroke clients across the community.
To maintain comprehensive and accurate records in line with policies.
