Norfolk Community Health and Care NHS Trust is seeking a Community Nurse for the West Norwich Community Nursing Team. You will assess and deliver care to patients with varied health issues in a home and community setting.
The role emphasises holistic, patient-centred assessments and individualised care plans, including diabetes management, wound care, catheter care, bowel care, and palliative/end-of-life support for those with complex needs.

- Holistic, patient centred assessments and individualised care.
- Diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management.
