East of England Community Health and Care NHS Trust in Wymondham is seeking a Band 5 Community Nurse on a 6-month fixed term contract to provide in-home care and assessments for patients. You will deliver palliative and wound care, supported by a multidisciplinary team, and contribute to holistic patient planning.

The role requires a registered nurse with NMC registration, a full driving licence, and the ability to work across a 7-day rota, including weekends, to meet patient needs.

Qualifications

• Registered nurse with palliative and wound care experience.
• Driving licence and access to vehicle required.
• Able to work across 7 day rota including weekends.
• Experience in community/home care settings preferred.

Responsabilités

• Assess, plan, implement and evaluate patient care.
• Develop individualised, evidence-based care plans.
• Liaise with GPs, Social Services and MDTs to coordinate care.
• Maintain records and ensure informed consent and safety.
