Job summary

Norfolk Community Health and Care NHS Trust is seeking a Community Nurse for the West Norwich Community Nursing Team. You will assess and deliver care to patients with varied health issues in a home and community setting.

The role emphasises holistic, patient-centred assessments and individualised care plans, including diabetes management, wound care, catheter care, bowel care, and palliative/end-of-life support for those with complex needs.

Job description

Community Nurse - West Norwich Community Nursing Team

The workload requires a good range of clinical skills to be applied in managing our patients with varied health problems including diabetes management, traumatic and post-operative wound care, pressure ulcer prevention and management, catheter management, bowel care, and palliative and end of life care. Many of our patients will have complex multiple pathologies and so we aim to provide holistic, patient centred assessments, and deliver individualised care.
