Job summary

West London NHS Trust is seeking a Community Nurse to join a patient-focused home-visit team delivering high-quality care within the local community. You will support staff, mentor colleagues, and contribute to patient and family education across a multi-professional environment.

The role requires a full driving licence and access to a car, with shifts from 8am to 8pm, including weekends, in a patient-home setting across the local authority area.

Qualifications

• Experience working in community setting/home visits.
• Proven ability to teach and assess mentees.
• Continued Professional Development (CPD) evidence.
• Strong communication with patients, families and partners.

Responsibilities

• Wound, catheter and IV therapy care as part of in-home visits.
• Medication/injection administration and diabetes management.
• Attend to patient needs and provide family advice.
• Collaborate with multi-disciplinary team and local authorities to support care pathways.
