Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced community nurse to assess, plan, deliver and evaluate care in patients’ homes, care homes and community clinics. You will prioritise wound healing, including lower limb ulcers, while addressing broader caseload needs with evidence-based practice.

The role requires NMC registration, post-preceptorship experience, and the ability to work autonomously within a multidisciplinary team.

Qualifications

• NMC registered nurses with post-preceptorship experience.
• Interest in community nursing, wound healing and lower limb management.
• Excellent clinical assessment and decision‑making skills.
• Ability to work autonomously within a multidisciplinary team.
• Commitment to continual professional development.
• Hold a full UK driving licence and have access to a vehicle.

Responsibilities

• Provide high-quality, evidence-based care in patients' homes, care homes and community clinic settings.
• Assess, plan, deliver and evaluate care for patients with acute, chronic and complex healthcare needs, promoting independence.
• Lead wound healing management including lower limb ulcers and other wounds.
• Support patients with medication management, insulin administration, long-term condition management and clinical monitoring as required.
