Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Registered Nurse for the Integrated Neighbourhood Team with a wound healing focus. You will assess, plan, deliver and evaluate care for patients in home, care homes and community clinics, coordinating treatment while supporting independence within diverse settings.

You will join a multidisciplinary neighbourhood team delivering evidence-based care, with emphasis on wound healing and lower limb management.

Qualifications

• NMC registered with post-preceptorship experience.
• Interest in community nursing and wound healing.
• Delivery of evidence-based care
• Hold a full UK driving licence and access to a vehicle.
• Immunisation and vaccination report from UK provider.
• Experience in wound care or lower limb management.

Responsibilities

• Assess, plan, deliver and evaluate care for patients at home or in community settings.
• Coordinate treatment plans and support wound healing and lower limb management.
• Provide holistic care including medication management and chronic condition care.
• Work within multidisciplinary teams and participate in professional development.
