A community health service provider is seeking a Community Health and Social Care Support Worker to join their team. What does the Community First Team do?
We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The person needs short term/interim support in their homes. The teams are committed to caring and supporting people at home and work over a seven day rota.

What are the hours?
Various shift patterns (including unsocial hours, weekends, and bank holidays).
Where would I be working?
Base: Florence Nightingale Community Hospital
Area Covered: Derby City and surrounding area
Working in this team means you will need to be able to meet the travel requirements of going to people's homes. This will require you have a UK driving license and access to a vehicle. DCHS has a mileage policy and car lease scheme available to support our staff.
The Purpose and Activities of DCHS?
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
