Job summary

Alzheimer's Society is seeking a Dementia Adviser in Worcestershire for 21 hours per week across the North region (Bromsgrove, Redditch & Wythall) on a permanent basis. You will work mainly in the community, visiting GP practices, conducting home visits and supporting people living with dementia and their carers.

You will travel between community locations, build trusted relationships, provide information and guidance, and help people access local support services to live well with dementia.

Qualifications

• Understanding of dementia or willingness to learn about experiences of people living with dementia.
• Experience providing information, guidance, advice or emotional support in health, social care or people-focused roles.
• Strong listening and communication skills to build trust with diverse people.
• Ability to organise workload, balance priorities and use support when needed.

Responsibilities

• Build trusted relationships with people living with dementia and carers.
• Help people maintain independence and wellbeing.
• Provide tailored information, advice and emotional support reflecting individual backgrounds.
• Support people via phone, in person, or home visits as appropriate.
• Take part in referral and telephone support rota and respond promptly to referrals.
• Maintain accurate, confidential records and manage workload.
• Help people access local services and community activities.
• Collaborate with colleagues to develop the service.
• Facilitate peer support groups and host drop-in sessions.
• Attend community events and promote dementia support locally.
