Surrey Downs Health and Care in Dorking is recruiting for a Band 4 Community Services coordinator. The role involves working with GPs, district nurses and care co-ordinators to co-ordinate patient care and provide navigation and support within the local health and care system.

Qualifikationen

• Car driving licence required.
• NVQ Level 3 in adult care or equivalent qualifications or working towards.
• Proficient IT skills including Outlook, Word, Excel and PowerPoint.

• Make and manage appointments for patients with primary, secondary and community care services.
• Help patients navigate the health and care system and coordinate care with the team.
• Identify when more support is needed and alert the appropriate clinical contacts.
• Maintain accurate records in GP and other systems, ensuring data protection compliance.
